Japanese Language Proficiency Test in Canberra

Forms: Please read the guidelines, fill out and submit all parts of the application form and be certain that all biographical data is correct. Be sure to attach the required photographs. Please note that when printing forms they must be printed single-sided as different forms will go to different places.
